How You’ll Help Transform Healthcare:
Provides therapeutic services to patients from young adult through advanced adulthood. Services include provision of initial assessment, individual, family and group therapy. Works as a member of the multidisciplinary team.
-Assesses patient’s need for non-medical services within the parameters of the treatment plan. Works as a member of the multidisciplinary treatment team.
-Provides individual, family and group therapy.
-Develops and maintains therapeutic relationships with patients in the individual, family and group setting.
-Coordinates the patient’s treatment with the multidisciplinary treatment team, patient’s family, referral source and aftercare provider. Makes appropriate discharge plans.
-Records patient data in the medical record to include the psychosocial assessment, group, family and individual progress notes and discharge plans/appointments.
-Works cooperatively with other members of the Clinical Programs Department and treatment team, provides coverage and arranges coverage when needed.
-Attends in-service and continuing education activities in order to comply with the system requirements and to maintain up to date license (LCSW, LPC).
What We Require:
Education: Masters in Social Work, Counseling or related field required.
Experience:. One to three years of related experience required.
Other Minimum Qualifications: Effective interpersonal, communication and teamwork skills required. Must successfully complete orientation and competency validation for position.
